(a) After the registrant's first birthday, given names for a child whose birth was recorded without given names may be added to the certificate upon written request of:

1. (1) Registrant, if of legal age;
2. (2) Both parents;
3. (3) The mother in the case of a child born out of wedlock;
4. (4) The father in the case of the death or incapacity of the mother;
5. (5) The mother in the case of the death or incapacity of the father; or
6. (6) The guardian or agency having legal custody of the registrant.

(c)

1. (1) After the first birthday, one (1) or more items of documentary evidence must be submitted to substantiate the name being added.
2. (2) This certificate shall be marked “amended”.
