(a) The Certificate of Cleanliness and Disinfection shall contain the following information:
- (1) The truck crates and equipment must be washed with soap and water under high pressure and disinfected at the place of origin as specified in 9 C.F.R. § 71.10 (1983); and
- (2) Certificate must be issued after each use prior to entry into the State of Arkansas and must be signed and dated by the federal or state veterinarian or his or her designee.
- (b) This certificate should also provide the following information: 1. Date and time of cleaning and disinfection; 2. Name and address of consignor and consignee; and 3. Other information. ________________________ Federal or State Veterinarian
