(a) Required qualifications.
(2) A license from another state is substantially similar to an Arkansas license if an applicant has, or the other state’s licensure qualifications require an applicant to have, passed an examination or examinations given by the National Certification Commission for Acupuncture and Oriental Medicine (NCCAOM) in either:

(2) As evidence that the applicant’s license from another jurisdiction is substantially similar to Arkansas’s, the applicant shall submit the following information:
(i) Evidence of current and active licensure in that state.
(ii) The Arkansas State Board of Acupuncture and Related Techniques may verify this information online or by telephone; and

(3) To demonstrate that the applicant has not had a license revoked for bad faith or a violation of law, rule, or ethics, as required by subdivision (a)(4) of this section, and that the applicant does not hold a license on suspended or probationary status, as required by subdivision (a)(5) of this section, the applicant shall provide the board with:
