(a) The Arkansas State Board of Acupuncture and Related Techniques shall authorize such forms as are necessary from time to time for the:
- (1) Application for licenses; and
- (2) Renewal of licenses.
- (b) The authorized forms may be reproduced without permission from the board but shall not be altered or changed in any way by any prospective licensee.
- (c) Authorized forms shall be available from the board to any member of the public or any prospective licensee.
- (d) Requests for forms may be made in writing or by telephone call to the board at its offices or to the Secretary of the Arkansas State Board of Acupuncture and Related Techniques.
