1. (a) Whenever any licensed pharmacist shall change his or her place of employment for any reason, it shall be the duty of the former and current employer and said licensed pharmacist to notify the Arkansas State Board of Pharmacy in writing of such change within five (5) days after such change of employment.

2. (b) Notification must:

   1. (1) Be made by:

      1. (A) Letter;
      2. (B) Fax;
      3. (C) Email; or
      4. (D) Through the board website; and
   2. (2) Contain the new place of employment of the licensed pharmacist and his or her license number.
