1. (a) Any person or persons seeking information respecting the Arkansas State Board of Pharmacy or desiring to submit complaints or charges thereto or make request thereof shall do so by filing with the board an instrument in writing:

   1. (1) Signed by the writer; and
   2. (2) Containing a return address.
2. (b) Communications need not be typed but should be legible.
