The fees are as follows:

   1. (A) Initial application for licensure by examination or by reciprocity: seventy-five dollars ($75.00).
   2. (B) An applicant whose application is rejected shall be refunded all but twenty-five dollars ($25.00) of the paid application fee;
2. (2) Application for temporary permit: thirty-five dollars ($35.00);
3. (3) Annual renewal: forty dollars ($40.00); and
4. (4) Reinstatement: all delinquent fees plus a penalty of ten dollars ($10.00) per year for all years delinquent.
