- (a) Any person who plans to practice as a licensed respiratory care practitioner in the State of Arkansas shall, in addition to demonstrating eligibility in accordance with the requirement of Arkansas Code § 17-99-302 or Arkansas Code § 17-99-303, apply for licensure to the Arkansas State Medical Board on forms and in such manner as the board shall prescribe.
- (b) Forms. Application forms may be secured from the board.
