Application for licensure

- (a) Any person who plans to practice as a licensed occupational therapist or occupational therapy assistant in the State of Arkansas shall, in addition to demonstrating his or her eligibility in accordance with the requirements of Section 7 of Acts 1977, No. 381, apply for licensure to the Arkansas State Medical Board, on forms and in such a manner as the board shall prescribe.
- (b) Forms. Application forms can be secured from the board.
(c) Filing requirements.
- (1) Completed applications shall be submitted together with necessary documents and filing fee to the board.
- (2) The filing fee is not refundable.
- (3) Applications and documentation must be completed within one (1) year of date of receipt by the board.
- (4) Applications and documentation over one (1) year old are voided and the applicant must reapply.
- (d) Board action on applicants. Applications for licensure shall be acted upon by the board no later than its next regularly scheduled meeting following the receipt of the required fee and all credentials.
