1. (a) An MA-C whose name is legally changed shall submit:

   1. (1) A name change request;
   2. (2) A copy of the marriage license or court action; and
   3. (3) The required fee.
2. (b) An MA-C shall immediately notify the Arkansas State Board of Nursing in writing of a change in mailing or residential address.
