The decision of the Arkansas State Board of Nursing shall contain:

1. (1) Findings of fact made by the board;
2. (2) Conclusions of law reached by the board;
3. (3) The order of the board based upon these findings of fact and conclusions of law; and

4. (4) A statement informing the person whose license is involved of:

   1. (A) His or her right to request a judicial review; and
   2. (B) The time within such request must be made.
