(c) Any program with a pass rate below seventy-five percent (75%) shall:

1. (1) First year:

   1. (A) Receive a letter of concern; and
   2. (B)

(i) Provide the board with a report analyzing all aspects of the program.

1. (ii) The report shall:

   1. (A) Receive a letter of warning; and

   2. (B)

      1. (i) Program director and parent institution representative shall appear and present a report to the board.
      2. (ii) The report shall identify and analyze:

1. (3) Third consecutive year:

   1. (A) Be placed on conditional approval; and

   2. (B) Conditional approval will be granted until:

      1. (i) Two (2) consecutive years of an above seventy-five percent (75%) pass rate is achieved; or
      2. (ii) The board withdraws approval status for noncompliance with the education standards.
