- (a) Transcripts of all students enrolled in the program shall be maintained according to policies of the parent institution.
- (b) Transcripts shall reflect courses taken.
(c) The final transcript shall include:
- (1) Dates of admission;
- (2) Date of separation or graduation from the program;
- (3) Hours/credits/units earned, degree, diploma, or certificate awarded;
- (4) The signature of the program director, registrar, or official electronic signature; and
- (5) The seal of the school or be printed on security paper or an official electronic document.
- (d) Current program records shall be safely stored in a secure area.
- (e) Permanent student records shall be safely stored to prevent loss by destruction and unauthorized use.
