1. (a) Institutional accreditation. The parent institution shall be approved by the appropriate state body.

2. (b) Institutional organization.

   1. (1) The parent institution shall be a:

      1. (A) Post-secondary educational institution;
      2. (B) Hospital; or
      3. (C) Consortium of such institutions.

   2. (2) The institutional organizational chart shall indicate lines of authority and relationships with:

      1. (A) Administration;
      2. (B) The program; and
      3. (C) Other departments.
   3. (3) The program shall have at least equal status with comparable departments of the parent institution.

3. (c) Program organization.

   1. (1) The program shall have a current organizational chart.
   2. (2) The program shall have specific current job descriptions for all positions.
