1. (a) All prescriptions issued by the APRN shall:

   1. (1) Contain the name of the client and the APRN’s:

      1. (A) Name;
      2. (B) Title;
      3. (C) Address;
      4. (D) Telephone number; and
      5. (E) Signature with the initials “APRN”; and
   2. (2) Include information contained in 17 CAR § 123-604(e).

2. (b)

   1. (1) All prescriptions for controlled substances shall be in accordance with federal rules.
   2. (2) The APRN’s assigned Drug Enforcement Administration registration number shall be included on the prescription when a controlled substance is prescribed.
