1. (a) The patient shall be evaluated through an in-person examination at least:

   1. (1) Every three (3) months by the APRN; and
   2. (2) One (1) time every six (6) months by a physician who is licensed by the Arkansas State Medical Board.

2. (b)

   1. (1) A current Prescription Drug Monitoring Program report shall be reviewed at least every six (6) months.
   2. (2) The review shall be documented in the patient’s medical record.

3. (c) A current pain contract with the patient shall be maintained and include, at a minimum, requirements for:

   1. (1) Random urine drug screens; and
   2. (2) Random pill counts.
