Chronic nonmalignant pain is defined as pain requiring more than three (3) consecutive months of prescriptions for:

1. (1) An opioid that is written for more than the equivalent of ninety (90) tablets, each containing five milligrams (5 mg) of hydrocodone;
2. (2) A morphine equivalent dose of more than fifteen milligrams (15 mg) per day; or
3. (3) Tramadol, an average dose of two hundred milligrams (200 mg) or greater per day.
