(a) The Arkansas State Board of Dental Examiners must be notified within ten (10) business days of:

1. (1) If the mobile dental facility is sold;
2. (2) Any change relating to dentists to whom patients are to be referred for follow-up care;
3. (3) Any change in the procedures for obtaining follow-up or emergency care; or
4. (4) Any changes of operator.

(c) The board must be notified within thirty (30) business days of:

1. (1) Any change of official business address or telephone number; or
2. (2) Cessation of operation.
