Application for licensure, reciprocity, and renewal

(a) Application for licensure.
(1)
- (A) Official application forms are available on the Arkansas Board of Podiatric Medicine’s website.
- (B) No application for licensure will be considered unless fully and completely filled out on this form.
- (2) An applicant must have official documentation sent to the Secretary-Treasurer of the Arkansas Board of Podiatric Medicine from the National Board of Podiatric Medical Examiners that a passing score was achieved by the applicant on the American Podiatric Medical Licensing Examination Part I and Part II, or successor exam, of the National Board Examination.
- (3) An applicant may take the state licensing examination or submit official documentation from the National Board of Podiatric Medical Examiners of a passing score on the Part III Examination (or successor exam).
- (4) No applicant will be admitted to the state examination except on presentation of a valid driver’s license or other form of government-issued photo identification.
- (5) Applications should be sent to the address provided on the Arkansas Board of Podiatric Medicine’s website.
(6)
- (A) An application fee of two hundred dollars ($200) shall accompany the application.
- (B) The fee shall be sent in form of:
(i) Cashier’s check;
(ii) Personal check; or
- (iii) Money order.
- (7) The applicant will furnish with the application transcripts under seal of the college or colleges of podiatric medicine attended.
- (8) If an applicant wishes to take the state examination, an application shall be completed and filed with the secretary-treasurer at least two (2) months prior to the Arkansas Board of Podiatric Medicine examination, unless otherwise provided by law.
- (9) The application is subject to Arkansas law at the time of consideration of the application by the Arkansas Board of Podiatric Medicine.
- (10) The state examination shall be conducted in English.
- (11) All state examinations shall be conducted in the presence of at least one (1) member of the Arkansas Board of Podiatric Medicine.
- (12) If an applicant fails to pass the state examination, the applicant may appear before the Arkansas Board of Podiatric Medicine for not more than two (2) examinations, both of which must be taken within a period of six (6) months from the date the applicant’s license application is denied for failure to pass the exam.
- (13) The Arkansas Board of Podiatric Medicine shall grant a license to an applicant who fulfills the Arkansas requirements for licensure and is a person who holds a federal Form I-766 United States Citizenship and Immigration Services-issued Employment Authorization Document, known popularly as a “work permit”.
(b) Reinstatement. An applicant who can demonstrate that he or she meets the following criteria may have his or her license reinstated upon payment of the application fee:
- (1) Was previously licensed as a podiatrist at any time in this state;
- (2) Held his or her license in good standing at the time of licensing;
(3) Did not have his or her license revoked for:
- (A) An act of bad faith; or
- (B) A violation of law, rule, or ethics;
- (4) Is not holding a suspended or probationary license in any state; and
(5) Furnish evidence of completion of the number of hours of acceptable continuing medical education computed by multiplying twenty (20) times the number of years the licensee has held an inactive or invalid license, not to exceed one hundred (100) hours.
- (c) Reciprocity.
(1) Required qualifications. An applicant applying for reciprocal licensure shall meet the following requirements:
(A)
- (i) The applicant shall hold in good standing a substantially similar license in another United States jurisdiction.
- (ii) A podiatric medical license from another state is substantially similar to an Arkansas podiatric medical license if an applicant has, or the other state’s licensure qualifications require an applicant to have:
- (a) (a) Graduated from a legally incorporated, regularly established school of podiatric medicine recognized by the Council on Podiatric Medical Education of the American Podiatric Medical Association; and United States jurisdiction; and
(b) (b) Completed the residency described in Arkansas Code § 17-96-303(d)(2).
- (iii) The applicant shall not have had a license revoked for:
- (a) (a) An act of bad faith; or
(b) (b) A violation of law, rule, or ethics.
(iv) The applicant shall not hold a suspended or probationary license in a medicine.
- (B) The applicant shall be sufficiently competent in the field of podiatric medicine.
(2) Required documentation.
(A) An applicant shall submit:
- (i) A fully executed application;
- (ii) The required fee; and
- (iii) The documentation described below.
(B) As evidence that the applicant’s podiatric medical license from another jurisdiction is in good standing and is substantially similar to an Arkansas podiatric medical license, the applicant shall submit the following information:
- (i)
- (a) (a) Evidence of current licensure in good standing in that state.
(b) (b) The Arkansas Board of Podiatric Medicine may verify this information online or by telephone;
(ii) A certificate of graduation from a legally incorporated, regularly established school of podiatric medicine recognized by the Council on Podiatric Medical Education of the American Podiatric Medical Association; and
- (iii) A certificate issued by an accredited podiatric residency program approved by the Council on Podiatric Medical Education of the American Podiatric Medical Association as proof of the satisfactory completion of the residency described in Arkansas Code § 17-96-303(d)(2).
(C) To demonstrate that the applicant has not had a license revoked for bad faith or a violation of law, rule, or ethics, as required by 17 CAR § 85-101(c)(1)(A)(iii), and that the applicant does not hold a license on suspended or probationary status, as required by 17 CAR § 85- 101(c)(1)(A)(iv), the applicant shall provide the Arkansas Board of Podiatric Medicine with:
- (i) The names of all states in which the applicant is currently licensed or has been previously licensed; and
- (ii)
- (a) (a) Letters of good standing or other information from each state in which the applicant is currently or has ever been licensed showing that the applicant’s license has not been revoked for the reasons listed in 17 CAR § 85-101(c)(1)(A)(iii) and is not on suspended or probationary status as described in 17 CAR § 85-101(c)(1)(A)(iv).
(b) (b) The Arkansas Board of Podiatric Medicine may verify information online or by telephone.
(D) As evidence that the applicant is sufficiently competent in podiatric medicine, an applicant shall:
(i) Submit official documentation from the National Board of Podiatric Medical Examiners of a passing score on the American Podiatric Medical Licensing Examination Part I and Part II (or successor exams); and
- (ii) Either:
- (a) (a) Pass the Arkansas state licensing examination; or
- (b) (b) Submit official documentation of a passing score on the PMLexis, Part III Examination (or successor exam).
