In order to receive payment from the Auctioneer Education and Recovery Fund, an aggrieved party must submit a request for payment, which request must contain:

1. (1) Amount requested to be paid;
2. (2) Name and address of payee;
3. (3) Complaint number; and
4. (4) Notarized, signed statement setting forth whether any amounts have been paid to the aggrieved party by licensee.
