(a) Applications for renewal shall include:

1. (1) Renewal form;
2. (2) Certification status form; and
3. (3) Physician direction form.

(b)

1. (1) Licenses must be renewed annually at a fee established by the Arkansas State Board of Athletic Training.

2. (2)

   1. (A) Unrenewed licenses become inactive as of a date established by the board.
   2. (B) To return to regular status, a reactivation fee must be paid.
3. (3) Licenses reactivated ninety (90) days after the renewal date will be assessed a late fee in addition to the renewal fee and the reactivation fee.

4. (4)

   1. (A) Individuals interested in being placed on inactive status must submit a written request to the board.
   2. (B) An athletic trainer on inactive status may not engage in the provision of athletic training services.

(c)

1. (1) The temporary nonrenewable athletic trainers permit will expire one (1) year after the date of application.
2. (2) The temporary permit fee will be paid quarterly.
3. (3) Any nonpayment of quarterly fees will revoke the nonrenewable temporary permit.
4. (4) The temporary permit fee will be established by the board.
