1. (a) A monthly reporting log, referred to as the “Caseload” and “Birth Log” (found in Appendix A or available on the Department of Health website), will be maintained and sent to the department postmarked no later than the tenth of each month regardless of any changes or additions to the log.
2. (b) Each woman receiving care for two (2) or more visits shall be listed on the Caseload and Birth Log in the following month of care, regardless of whether or not the LLM attended the birth.

Codification Notes: "LLM" means licensed lay midwife.
