1. (a) Apprentices who hold a valid permit prior to the effective date of this part will follow the requirements found in Appendix B: Transitional Provisions and Forms.

2. (b)

   1. (1) An LLM will be responsible for notifying the Department of Health of any apprentices accepted under their supervision within thirty (30) days of signing, but prior to the apprentice providing any services.

   2. (2)

      1. (A) The department Preceptor-Apprentice Agreement form (found in Appendix A or available on the department website) shall be used for this notification.
      2. (B) Preceptors must meet all NARM preceptor requirements.
   3. (3) Any changes in the apprentice’s contact information must be provided to the department by the LLM within thirty (30) days of the status change.
   4. (4) If the apprentice is still under the LLM’s supervision after three (3) years, the LLM must complete a new form indicating this status.

3. (c)

   1. (1) Should the Preceptor-Apprentice Agreement be terminated by either party, it is the responsibility of both parties to notify the department immediately.
   2. (2) An apprentice must not continue to perform under any preceptor or preceptors unless a new signed Preceptor-Apprentice Agreement is on file with the department.
   3. (3) A signed Preceptor-Apprentice Agreement for every preceptor under whom an apprentice trains must be signed and sent to the department.
4. (d) Apprentices shall follow all applicable Arkansas laws and this part.
5. (e) Apprentices are required to comply with all provisions of HIPAA (Health Insurance Portability and Accountability Act of 1996, Pub. L. No. 104-191).
