(2) Any person doing business in this state, or any cemetery, funeral establishment, corporation, partnership, joint venture, voluntary or religious organization, or any other entity is prohibited from:
- (A) Maintenance or operation of a building or structure within this state as a crematory without a license or in violation of this part of the State Board of Embalmers, Funeral Directors, Cemeteries, and Burial Services or laws or rules of other applicable regulatory bodies;
- (B) Holding oneself out to the public as a crematory authority without being licensed under this part or performing a cremation without a cremation authorization form signed by the authorizing agent;
- (C) Signing a cremation authorization form with the actual knowledge that the form contains false or incorrect information;
- (D) Performing a cremation in other than a licensed crematory authority; and
- (E) Violation of any other provision of this part.
