- (a) Four (4) types of funeral establishment licenses shall be issued by the State Board of Embalmers, Funeral Directors, Cemeteries, and Burial Services.
(b) These establishment licenses are:
- (1) Funeral Establishment Type A-Full-Service Funeral Firm;
- (2) Funeral Establishment Type B-Mortuary Service Firm;
- (3) Funeral Establishment Type C-Crematory Authority; and
- (4) Funeral Establishment Type D-Transport Service Firm.
- (c) All establishment renewals are due and payable on or before December 31, regardless of whether they renew annually or biennially.
