(a) The entrance examination shall be given by:

1. (1) Members of the State Board of Optometry; or
2. (2) Any duly qualified individual or committee designated by the board.

(b) Such examination may be oral or written, or both, and shall consist of:

1. (1) The subjects designated in Acts 1941, No. 94, § 5, as amended by:

   1. (A) Acts 1957, No. 102;
   2. (B) Acts 1979, No. 710;
   3. (C) Acts 1985, No. 875;
   4. (D) Acts 1991, No. 397;
   5. (E) Acts 1987, No. 101; and
   6. (F) Acts 1997, Nos. 176 and 186; and
2. (2) Other subjects designated by the board.

(c)

1. (1) Upon successful completion of the entrance examination an applicant must submit:

   1. (A) An application for initial licensure; and
   2. (B) A fifty-dollar fee.

2. (2) The board shall waive the initial licensure fee if the applicant:

   1. (A) Is receiving assistance through the:

(i) Arkansas Medicaid Program;

1. (ii) Supplemental Nutrition Assistance Program;

   1. (iii) Special Supplemental Nutrition Program for Women, Infants, and Children;

      1. (iv) Temporary Assistance for Needy Families Program; or
      2. (v) Lifeline Assistance Program;
   2. (B) Was approved for unemployment within the last twelve (12) months; or

   3. (C) Has an income that does not exceed two hundred percent (200%) of the federal poverty income guidelines.

      1. (d) The board shall grant a license to an applicant who fulfills the Arkansas requirements for licensure and is a person who holds a federal Form I-766 United States Citizenship and Immigration Services-issued Employment Authorization Document, known popularly as a “work permit”.
