(a)
- (1) Within fourteen (14) days after the changing of a legal name, the licensee, credential holder, or commission holder shall notify the Director of the Division of Arkansas State Police in writing of the change and provide court documentation that officially created the change.
- (2) A Division of Arkansas State Police form shall be provided for that purpose.
- (b) If the person would like a new license or credential card printed with updated information, he or she may destroy the old document and apply for a replacement with payment of a fee.
