(a) No later than June 1 of each year of the permit term, the operator shall submit to the Division of Environmental Quality an annual report that contains the following information:

1. (1) A site map showing the current status of the permit area with the following features clearly marked:

   1. (A) Permit boundary;
   2. (B) Equipment installation;
   3. (C) All roadways including entrance road;
   4. (D) Water impoundments and water circulation system, if applicable;
   5. (E) Areas mined but unreclaimed;
   6. (F) Areas currently being mined;
   7. (G) Areas being reclaimed;
   8. (H) Area not yet affected; and
   9. (I) The section, range, township, and county; and

2. (2) A narrative providing the following:

   1. (A) The total number of affected acres that have not yet been released by the division; and
   2. (B) The amount of material mined during the period from May 1 of the previous year through April 30 of the current year.

(b) As authorized in Arkansas Code § 15-57-319(c), the Arkansas Pollution Control and Ecology Commission prescribes the following annual permit fee on affected land:

1. (1) First one hundred (100) acres, ten dollars ($10.00) per acre;
2. (2) One hundred one (101) to two hundred (200) acres, seven dollars and fifty cents ($7.50) per acre; and
3. (3) Two hundred one (201) acres and up, five dollars ($5.00) per acre.
