(b) The applicant shall disclose:

1. (1) The quantity of water to be diverted for direct use;
2. (2) The quantity of water to be stored away from the point of diversion when needed;
3. (3) The total amount of water to be diverted;
4. (4) The proposed time or times of diversion;
5. (5) The purpose for which the water is to be diverted;
6. (6) The location of the land on which the water is to be used;
7. (7) The proposed conservation plan;

8. (8) If for irrigation:

   1. (A) The area and legal description of the lands irrigated; and
   2. (B) The kinds of crops to be cultivated under irrigation during the water year; and
9. (9) Any other reasonable information requested by the director.
