If approved, the Director of the Arkansas Natural Resources Commission shall issue a permit and shall designate thereon:
- (1) The amount of water permitted;
- (2) The authorized use;
- (3) The point of approved diversion;
- (4) A legal description of the land of intended use; and
- (5) Approval of the conservation plan.
