The reports shall contain information requested on the attached, or revised, forms:

1. (1) For water used for agriculture:

   1. (A) Number and size of diversions;
   2. (B) Name, address, and phone number of water user;
   3. (C) Type of crops, livestock, poultry, or fish grown;

   4. (D) Acreage:

      1. (i) Irrigated; or
      2. (ii) Aquacultured;
   5. (E) Quantity of water used; and

   6. (F) Location of the:

      1. (i) Diversions, including real estate tax bill parcel number; and
      2. (ii) Water use;

2. (2) For water used for other than agriculture:

   1. (A) Number, size, and location of diversions;
   2. (B) Name, address, and phone number of water user;
   3. (C) Use made of the water; and
   4. (D) Quantity of water used; and
3. (3) Other information deemed necessary by the Arkansas Natural Resources Commission.
