(b) A certified nutrient applicator shall maintain the following application records for a period of not less than five (5) years from the date the application was made, including:

1. (1) Type and amount of nutrients applied by field;
2. (2) The source or sources of nutrients applied by field;
3. (3) The location of the field or fields where nutrients were applied;
4. (4) The date of application by field;
5. (5) The application rate used by field;
6. (6) The name and address of the person or business for whom the nutrients were applied; and
7. (7) The cover vegetation by field.
