Where financial assistance is sought in connection with an existing or proposed reservoir, the following additional information must be submitted with the application:

1. (1) The application should contain a description of all:

   1. (A) Estimated firm annual yields; and
   2. (B) Proposed reservoir storage capacities;

2. (2) A statement containing the:

   1. (A) Proposed purposes for which water will be stored or used; and
   2. (B) Places of use or potential use for such water;
3. (3) A statement of the relative allocation of project costs to each project purpose if for more than one (1) purpose; and
4. (4) A brief description of existing or proposed improvements in the project or project area and a description of all such improvements that may require relocation.
