(NAME) MUSEUM CLAIM OF OWNERSHIP OF LOAN TO MUSEUM
The (name) Museum seeks the owner(s) of property described herein for resolution of the loan agreement and/or return of the property to the owner(s):

Museum records indicate that the last known owner of the above-described property is (name and address if available). It shall be the responsibility of the owner of this loan to provide sufficient proof of his or her ownership. The loan shall be considered abandoned property and shall become the property of the (name) Museum if the lender fails to make a timely objection.
