2. (b) Pursuant to Arkansas Code § 12-12-917(f)(4)(B), the offender fact sheet will include the following information, as applicable:

   1. (1) Registration information as required in Arkansas Code § 12-12-908;
   2. (2) Risk level;
   3. (3) Date of deoxyribonucleic acid (DNA) sample;
   4. (4) Psychological factors likely to affect sexual control;
   5. (4) Victim age and gender preference;
   6. (5) Treatment history and recommendations; and

   7. (6) Other relevant information deemed necessary by the Sex Offender Assessment Committee or by professional staff performing sex offender assessments including, as available:

      1. (A) A recent photo of the offender/date of photo, if known;
      2. (B) Offender name;
      3. (C) Known aliases;
      4. (D) Social Security number;
      5. (E) Automated fingerprint ID class/code;
      6. (F) System identification number;
      7. (G) CSN#;
      8. (H) Division of Correction number;
      9. (I) Driver’s license number;
      10. (J) Date of birth;
      11. (K) Height;
      12. (L) Weight;
      13. (M) Race;
      14. (N) Identifying marks or scars;
      15. (O) Hair color;
      16. (P) Eye color;
      17. (Q) Community notification level;
      18. (R) Date of DNA sample;
      19. (S) Anticipated legal address/date last verified;
      20. (T) Temporary address, if applicable;
      21. (U) Probation/parole office;
      22. (V) Place of employment;
      23. (W) Brief description of crime;
      24. (X) Criminal history/sexual violence;
      25. (Y) Victim preference; and
      26. (Z) Treatment history.
