1. (a)

   1. (1) While conducting the assessment, a SOCNA staff person will be assigned to:

      1. (A) Monitor offenders;
      2. (B) Complete the SOCNA Sex Offender Community Notification Disclosure Form and the Arkansas Crime Information Center Acknowledgement form; and
      3. (C) Instruct them about basic assessment information.

   2. (2)

      1. (A) If the offender refuses to sign the forms, the SOCNA staff person will attempt to obtain a witness to document the offender’s refusal.
      2. (B) Both witnesses will sign and date the form.

2. (b)

   1. (1) SOCNA will make attempts to ascertain whether or not an offender’s DNA has been obtained prior to an interview at the SOCNA offices.
   2. (2) If evidence does not exist that DNA has been gathered, SOCNA will obtain a DNA sample.

3. (c)

   1. (1) Whenever possible, a digital photograph will be taken of each offender during the assessment process.
   2. (2) The offender will be identified at the time the photographs are taken.
   3. (3) The photos will be transferred to the offender fact sheet.

   4. (4) In the event that an offender is not photographed at the time of his or her interview, a photograph will be obtained from another source, such as the:

      1. (A) eOMIS system;
      2. (B) Local law enforcement;
      3. (C) Division of Community Correction; or
      4. (D) Arkansas Crime Information Center.
