Searches are conducted in these circumstances only:

1. (1)

   1. (A) A juvenile may be required to surrender his or her clothing, undergo an anal or genital bodily cavity search, and submit to a search only if there is probable cause to believe he or she is concealing contraband.
   2. (B) A body cavity search may be conducted only by a licensed medical professional; and

2. (2)

   1. (A) All special incidents including but not limited to the taking of hostages, escapes, assaults, staff use of restraint devices, and physical force are reported in writing to the director.
   2. (B) The report is also placed in the file of the juvenile concerned.
