The facility administration provides twenty-four-hour emergency medical and dental care availability as outlined in a written plan which includes:

1. (1) Arrangements for the emergency evacuation of the juvenile from the facility;
2. (2) Arrangements for the use of an emergency medical vehicle;
3. (3) Arrangements for the use of one (1) or more designated hospital emergency rooms or other appropriate health facilities; and
4. (4) Arrangements for emergency on-call physician and dental services when the emergency health facility is not located in a nearby community.
