An application for registration or renewal of registration as an athlete agent must be accompanied by a fee in the following amount:

1. (1) Two hundred dollars ($200) for an initial application for registration;
2. (2) One hundred dollars ($100) for registration based on a certificate of registration issued by another state;
3. (3) One hundred dollars ($100) for an application for renewal of registration; or
4. (4) One hundred dollars ($100) for renewal of registration based on a renewal of registration in another state.
