The petition shall state that those persons signing the petition request an election be held in the district to determine whether or not the citizens and property in the district shall be subject to the planning and land use control jurisdiction of the board created pursuant to this part. The petition shall include all of the following:

1. (1) The full name and address of each person signing the petition.
2. (2) The location of the property in the district owned by the person signing or by the entity on whose behalf the person is signing.
3. (3) The name and address of the person or persons submitting the petition to the Probate Court of Mobile County.
