Any proposed contracts between the corporation and dentists participating in the plan shall:

1. (1) List the responsibilities incurred by each party;
2. (2) Delineate the procedures for the remuneration of dentists for services provided thereby;
3. (3) Define and delineate the legal liabilities of the parties to one another, to subscribers, and to other affected persons; and
4. (4) Set forth the circumstances and procedures for terminating the contract.
