The provisional ballot officer shall give the provisional voter the following documents, all prescribed by the Secretary of State:

1. (a) a provisional verification statement,
2. (b) a provisional ballot,
3. (c) a provisional ballot inner envelope,
4. (d) a provisional ballot outer envelope,
5. (e) a voter reidentification form, and
6. (f) an instructional sheet.
