1. (1) Any person may appeal to the Commissioner for a hearing to challenge any of the following actions:

   1. (a) Denial of any certificate.
   2. (b) Revocation of any certificate.

   3. (c) Suspension of participation privileges.

      In all cases, the appeal must be submitted to the Director in writing within 30 days of the date the action or decision was made.
