The Certificate of Cleaning and Disinfection shall contain the following information:

1. (a) The truck, crates and equipment must be washed with soap and water under high pressure and disinfected at the place of origin as specified in 9 C.F.R. 1983, §71.10.

2. (b) A certificate must be issued after each use prior to entry into the State of Alabama and must be signed and dated by the state or federal animal health official or a practicing accredited veterinarian. This certificate should provide:

   1. 1. Date and time of cleaning and disinfecting.
   2. 2. Name and address of consignor and consignee.
