A person who has previously registered may submit in writing a revocation of his intent to claim paternity registration.

1. (1) Written revocation should be notarized.

2. (2) Upon receipt of a non-notarized written revocation request, the Department of Human Resources may require verification of the request from a putative father. That verification may include, but is not limited to, the following:

   1. (a) picture identification.
   2. (b) social security number.
   3. (c) date of birth.
3. (3) Upon receipt all previously recorded information in the Putative Father Registry will be destroyed.
