1. (1) Content of Notice. The notice of the hearing must include:

   1. (a) Date, time, place, and nature of the hearing;
   2. (b) Statement of legal authority and jurisdiction;
   3. (c) Reference to particular sections of the statutes and rules involved; and
   4. (d) A short, plain statement of the matters asserted or issues involved.

2. (2) Service of Notice. Service of notice of hearing may be:

   1. (a) By certified mail, return receipt requested;
   2. (b) By any sheriff, constable, or other officer authorized to make service of process in civil court proceedings in this state; or
   3. (c) By any employee, agent, or representative of the County or State Department of Human Resources; or
   4. (d) By mailing by first-class mail, postage prepaid, to the person to be served together with an acknowledgment of service form on a postcard, or in letter form with a return envelope, postage prepaid, addressed to the sender; or
   5. (e) By mailing by first-class mail, postage prepaid.
