The following are the fees, payable to the ASBOTF, which shall be collected by the Board:

1. (a) Initial license fee: Occupational Therapist $140.00; Occupational Therapy Assistant $115.00
2. (b) Limited permit fee: $25.00
3. (c) Renewal of license fee: Occupational Therapist $140.00; Occupational Therapy Assistant $115.00
4. (d) Late renewal fee: $50.00
5. (e) The cost of conducting a hearing of any person whose license is suspended, revoked or refused as a result of such hearings: Actual Cost
