Each applicant must request that a letter of good standing be issued by each state that has previously issued said applicant a license. Letter(s) must state that applicant’s license has/has not been suspended, revoked, probated, or otherwise voided. Said letter of good standing shall be sent directly to the Alabama State Board of Occupational Therapy.
