- (1) Licenses shall be valid for a period of 24 months.
- (2)
- (3) A Licensed Midwife's license expires on the last day of the month 24 months from when the license was granted.
- (4) Licenses that are not renewed as required will be invalid.
(5) Each Licensed Midwife is solely responsible for compliance with the requirements for license renewal. A Licensed Midwife's application for license renewal must include the following:
- (a) a completed license renewal application form which shall require the provision of the preferred mailing address and telephone number, and a statement of all felony offenses for which the licensee has been convicted;
- (b) proof of current Certified Professional Midwife certification from NARM, on file;
- (c) Proof of current professional liability insurance of no less than one hundred thousand dollars ($100,000) per occurrence and three hundred thousand dollars ($300,000) aggregate, on file;
- (d) Background check form and applicable fee; and
- (e) the renewal fee;
- (f) annual statistics forms; on file.
