- (1) This license limits the license holder to providing services in the specific area for which the License has been issued.
(2) Prior to and not later than March 15th, applicants for initial license shall submit all of the following:
- (a) Application for licensure,
- (b) The payment of a nonrefundable fee,
- (c) Documentation of GED or high school diploma,
(d) Documentation of one of the following:
- 1. A current RID certified membership card, or
- 2. EIPA performance level 4.0 – 5.0 exam and EIPA written exam and college degrees, or
- 3. BEI master or level V.
- (e) Proof of citizenship and,
- (f) Proof of citizenship form,
